Authorization and Release

I hereby authorize Locust Hills or its affiliates or assignees (collectively "Locust Hill"), to verify my past and present employment earnings records, bank accounts, stock balances, and any other asset balances that are needed to ascertain my credit worthiness. I hereby authorize Locust Hill to order a consumer credit report and verify other credit information, including my past and present mortgage and landlord references, and to perform a criminal background check. Authorization is further granted to the reporting agency to use a photo static reproduction of this authorization to obtain any information regarding the above-mentioned information. PRIVACY ACT NOTICE: This information is to be used by the agency collecting it or its assignees in determining whether you qualify as a prospective tenant under the Residential Rental Agreement pursuant to your request. It will not be disclosed outside the agency except as required or permitted by law. You do not have to provide this information, but if you do not, your application for approval may be delayed or rejected. This authorization has been provided to Locust Hill with the understanding that the applicant's credit profile, application documents and all materials or other information obtained in the application and underwriting process will be used (a) only by Locust Hill, its affiliates and related entities, and such other entities or individuals as may acquire (or contemplate the acquisition of) an interest in the home being rented or leased; and (b) only for purposes relating to such transaction or contemplated transaction.
